Upon acceptance of the completed application, ASME will noufy the applicant of the dates and lcxaiaons of scheduled examinations.
4.3 Op.rator C.rtification
A high capacity fossil fuel tired operator certilicate will he issued when the tollowrng requiren)ents are met:
(a) Ccrtihcation qualihcations (or the class dcsigna tions outlined in para 2.3;
(b) Successful completion of an operator certihcation examination as outlined in Section 3 of this Standard
4.3.1 Thc ccrtihcatc shall expire live (5) years atter its issue unless renewed.
4.3.2 lach ccrtihcate shall contain the tollowing minimum information:
(a) ldcntihcaiion as a high capacity fossil fuel hred operator certihcatc
(b) Class designation of cerhtication in accordance with para. 2.:
(c) Certitkd individual’s full name;
(d) Eflective da of certilication,
(e) Expiration date of certilication;
(f) Serial number to assure traceability and accuracy;
(g) Signature ot a designated ASML rcprcscntaIivc
